Lifeline Program Documents Checklist
1. Proof of Program Participation
If you qualify through a government assistance program (like Medicaid, SNAP, SSI, WIC, or Federal Public Housing), provide an official letter or statement that includes:
- Your name
- The program name
- Issuing agency
- A recent date (within the last 12 months or showing current eligibility)
2. Proof of Household Income
If you qualify based on income, acceptable documents include:
- Three consecutive pay stubs
- Prior year’s tax return
- Social Security benefits statement
- Unemployment or Veterans’ benefits letter
3. Proof of Date of Birth
Acceptable documents include:
- Government-issued ID (license or passport)
- Birth certificate
- Permanent resident card
4. Proof of Social Security or Tribal ID Number
Submit the first page of your SSN card, a W-2, or any government document showing the last four digits of your SSN or your Tribal ID.
5. Proof of Address
Documents must show your name and current residential address, such as:
- Utility bill
- Lease or mortgage statement
- Official government correspondence
6. Duplicate Address Verification
If multiple applicants live at the same address, you’ll need to complete a one-per-household worksheet to confirm independent eligibility.

8. Emancipated Minor
